All of these forms are replicas of those forms available in the church office. They are now available to you online. Please be aware that these forms must be submitted in person to the church office. Forms returned to the church electronically will not be accepted . Here is the proper procedure for using the forms online:
1. If you do not currently have Acrobat Reader installed on your computer, download it by clicking on the Acrobat logo below. This is not a complicated process. It should not take long.
2. Locate the church form you need below.
3. Click on the form's link. This will automatically start a download of the requested form and open the form.
4. Print the form to your printer using the print command within Acrobat Reader.
5. Complete the form using pen. IF A ROOM SETUP IS REQUIRED, YOU MUST ALSO PRINT OUT THE APPROPRIATE ROOM LAYOUT AND ILLUSTRATE BY PEN OR PENCIL THE DESIRED SETUP OF TABLES, CHAIRS AND ALL OTHER FURNITURE/SUPPLIES.
6. Bring all necessary forms to the church office and submit to the appropriate ministry assistant.
